Waiver and Release of Liability

I understand that my [or my child's] participation at World Body Boxing Association (AKA Scarberian Boxing Club) located 2250 Midland Ave. Unit 29, Scarborough, ON M1P4R9 will require me [or my child] to engage in physical activities that involve risks that may include physical injury. I knowingly and voluntarily agree to assume all of the risks associated with my [or my child's] participation and voluntarily hold harmless, and waive any and all rights to officers, employees, instructors, students, or agents. I intend this Agreement, Assumption of Risks, and Waiver to be binding upon any and all of my parents, children, present and former spouses or life partners, heirs, assigns friends, personal representatives, and estate in perpetuity. I consent to the above and give permission for my [or mu child's] photograph and/or video footage to be used in the future promotions.
